Individual or Caregiver (Supporting Individual through the survey) Survey to Understand the Individual's Daily Activity Limitations and Barriers
Rebecca Benson is our intern and an occupational therapy student at the University of South Dakota. She is seeking information on daily activity limitations and barriers for individuals with disabilities from their perspective. She will compare these community members' perceptions to understand where the differences lie. The survey is quick and short and should take around 5 minutes and up to 10 minutes to complete.
